Our vision is to provide seamless, outcome-focused rehabilitation and care oversight for people living with brain and spinal cord injuries, delivered through independent case management that always prioritises the client’s best interests.

How We Help.

As a small, personable, and highly supportive team, we were founded to bring clarity and coordination to a system that can often feel fragmented and overwhelming. Too often, families and professionals face the stress of fragmented services, duplicated processes, and uncertainty over who is responsible for what. We were established to change that – to bring clarity, reduce stress, and make sure that clients with complex injuries not only receive the right rehabilitation, but also safe, compliant day-to-day care when it’s needed.

Although LP Case Management is a young company, approaching its second year, it is built on decades of combined experience. Our background spans nursing, case management, expert reporting, and care governance, and our name reflects a commitment to strong professional leadership, regulatory compliance, and above all, client-centred outcomes.

Claire supports client scheduling and care team operations, ensuring smooth administration and the seamless day-to-day delivery of services.
